Abnormal liver enzymes should be approached as a pattern-recognition problem, not as a search for a single “liver function test.” Alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP) and gamma-glutamyl transferase (GGT) are principally markers of injury or cholestasis; albumin, prothrombin time/international normalized ratio (INR) and, in context, bilirubin provide information about hepatic synthetic or excretory function. The first clinical task is therefore to confirm the abnormality, identify the pattern, assess severity and synthetic function, and then choose a targeted differential diagnosis.
Step 1: confirm that the abnormality is real
Before launching an extensive work-up, repeat or verify an unexpected abnormal test when clinically safe to do so. Laboratory error, transient illness, strenuous exercise and non-hepatic sources can all produce misleading results. If ALP is elevated in isolation, GGT can help determine whether the source is hepatobiliary rather than bone. Bilirubin should be fractionated into conjugated and unconjugated components when the elevation is isolated or diagnostically important.
Do not call every abnormal chemistry a “liver function test.” A patient can have very high ALT from acute hepatocellular injury and still have preserved synthetic function. Conversely, a patient with advanced cirrhosis may have near-normal aminotransferases but impaired albumin synthesis, portal hypertension and reduced hepatic reserve. This distinction is central to both diagnosis and urgency.
Step 2: identify the pattern of injury
The most useful initial categories are hepatocellular, cholestatic and mixed. A hepatocellular pattern is dominated by AST and ALT elevation. A cholestatic pattern is dominated by ALP, often with conjugated hyperbilirubinemia. A mixed pattern contains substantial elevations of both.
The R-value provides a standardized way to classify the pattern, particularly when drug-induced liver injury is being considered:
R = (ALT / upper limit of normal ALT) divided by (ALP / upper limit of normal ALP).
An R-value of 5 or greater supports a hepatocellular pattern; 2 or less supports a cholestatic pattern; values between 2 and 5 indicate a mixed pattern. The calculation does not establish the cause. It organizes the next step.
Step 3: determine severity and urgency
The magnitude of an aminotransferase elevation does not reliably measure fibrosis or chronic disease severity, but very high values narrow the acute differential and can signal a medical emergency. Severe aminotransferase elevation raises concern for ischemic hepatitis, acute viral hepatitis, toxin or drug injury, autoimmune hepatitis and acute biliary obstruction, among other causes.
Urgency depends on more than the enzyme level. New encephalopathy with acute liver injury and coagulopathy should trigger immediate consideration of acute liver failure and early discussion with a transplant centre. Rising INR, hypoglycemia, rapidly increasing bilirubin, hemodynamic instability, severe systemic illness or a rapidly evolving clinical picture also require escalation.
Step 4: take a liver-specific history
A good history is often more valuable than an indiscriminate laboratory panel. Ask about alcohol quantity and pattern, metabolic risk factors, prescription drugs, recent antibiotics, acetaminophen, herbal and dietary supplements, recreational drugs, occupational exposures, viral hepatitis risk, travel, pregnancy, autoimmune disease, family history and previous abnormal results.
Drug-induced liver injury deserves deliberate attention because patients frequently omit supplements unless asked directly. The timing of exposure, latency, improvement after withdrawal and known hepatotoxicity profile all matter. Do not assume that a “natural” product is hepatically safe.
Step 5: choose the initial laboratory and imaging set
AASLD's practical 2025 framework recommends an initial evaluation that commonly includes complete blood count, AST/ALT, ALP, bilirubin, PT/INR, viral hepatitis testing, iron studies and abdominal ultrasound, with additional testing based on the degree and pattern of abnormality. In a stable patient with mild aminotransferase elevation, this basic framework often identifies or meaningfully narrows common causes. Moderate or persistent abnormalities warrant expansion to autoimmune, metabolic or other disease-specific testing.
For a hepatocellular pattern, the differential includes metabolic dysfunction-associated steatotic liver disease (MASLD), alcohol-associated liver disease, viral hepatitis, autoimmune hepatitis, drug-induced liver injury, hemochromatosis, Wilson disease in the appropriate age group, alpha-1 antitrypsin deficiency, celiac disease, muscle injury and ischemic injury. The exact panel should reflect the patient's age, context and pre-test probability.
For a cholestatic pattern, confirm hepatic origin when needed and start with ultrasound to look for biliary ductal dilation, gallstones, masses and gross structural disease. If extrahepatic obstruction remains possible, magnetic resonance cholangiopancreatography, endoscopic ultrasound or endoscopic retrograde cholangiopancreatography may follow depending on urgency and therapeutic need. If imaging does not show obstruction, consider intrahepatic cholestatic disease, including primary biliary cholangitis, primary sclerosing cholangitis, medication-related disease and infiltrative disorders.
Step 6: assess fibrosis separately from inflammation
One of the most important conceptual updates in hepatology is that the enzyme level and fibrosis stage answer different questions. In MASLD, for example, fibrosis stage is a much stronger determinant of liver-related outcome than the degree of steatosis or the height of ALT alone. A patient with normal or mildly elevated ALT can still have clinically significant fibrosis.
Non-invasive fibrosis assessment should therefore be considered independently when chronic liver disease is plausible. FIB-4 is a simple first-line risk-stratification tool using age, AST, ALT and platelet count. It is not a diagnostic test and performs differently across age groups and clinical settings. Patients above an appropriate threshold or with discordant clinical features may need transient elastography, other validated fibrosis tests, specialist evaluation or, in selected cases, biopsy.
What does AST greater than ALT mean?
The AST:ALT ratio is a clue, not a diagnosis. Alcohol-associated hepatitis often produces AST greater than ALT, frequently with a ratio above 2, but AST can exceed ALT in cirrhosis of many causes because of reduced hepatic clearance and other mechanisms. AST is also present in skeletal muscle and other tissues. The ratio must therefore be interpreted with the absolute enzyme values and clinical context.
What does an isolated ALP elevation mean?
First ask whether the ALP is hepatic. GGT or ALP isoenzyme testing can help. If the source is hepatic, ultrasound is usually the first structural test. A dilated biliary tree shifts attention toward extrahepatic obstruction; a non-dilated tree makes intrahepatic cholestatic disease more likely, although early or partial obstruction can still be present.
What does an isolated bilirubin elevation mean?
Fractionation is essential. Predominantly unconjugated hyperbilirubinemia suggests increased bilirubin production, reduced uptake or impaired conjugation; hemolysis and Gilbert syndrome are common considerations. Predominantly conjugated hyperbilirubinemia points toward impaired excretion from hepatocellular disease or cholestasis and should be integrated with ALP, aminotransferases and imaging.
A practical algorithm
- Confirm the abnormal result and identify whether the abnormality reflects injury, cholestasis or synthetic dysfunction.
- Classify the pattern as hepatocellular, cholestatic or mixed; use the R-value when helpful.
- Look immediately for urgency: encephalopathy, coagulopathy, rapidly rising bilirubin, hemodynamic instability or severe acute injury.
- Take a structured exposure history, including alcohol, prescriptions, acetaminophen, antibiotics, herbs and supplements.
- Use targeted first-line laboratory testing and abdominal ultrasound rather than an indiscriminate panel.
- Assess fibrosis separately when chronic liver disease is plausible; do not use ALT height as a fibrosis surrogate.
- Refer or escalate when the cause remains unclear, abnormalities persist, fibrosis risk is significant, or acute liver failure is possible.
Common errors to avoid
A normal ALT does not exclude advanced chronic liver disease. A very high ALT does not tell you the fibrosis stage. An AST:ALT ratio is not a stand-alone diagnosis of alcohol-related disease. An elevated ALP is not necessarily hepatic until its source is clarified. And INR should not be interpreted as a measure of liver function without accounting for warfarin, vitamin K deficiency and other confounders.
What should trainees remember?
Think in two parallel tracks. The first is “What pattern of injury is present and what could cause it?” The second is “How well is the liver functioning, and how urgent is the situation?” Those tracks overlap but are not interchangeable.
For practising clinicians, the highest-value sequence is confirmation, pattern recognition, severity assessment, focused serology/imaging and fibrosis risk stratification. That approach reduces unnecessary testing while identifying the patients who need urgent hepatology or transplant-centre evaluation.
