How NBI changes the image
Hemoglobin preferentially absorbs the blue/green wavelengths used by NBI, so superficial vessels appear darker and mucosal patterns gain contrast. The result is not magnification by itself; rather, NBI improves the contrast of structures already near the surface.
Barrett's esophagus
Dysplasia can manifest as subtle irregularity in mucosal or vascular pattern. High-quality surveillance increasingly combines HD white light with virtual chromoendoscopy such as NBI, followed by targeted sampling/resection of visible abnormalities and systematic Seattle-protocol biopsies where appropriate.
IBD surveillance
With HD colonoscopy, virtual chromoendoscopy is an accepted alternative to dye spraying in current surveillance guidance. It can help identify subtle lesion borders and direct targeted biopsies, although performance depends on inflammation control, preparation and operator experience.
Colorectal lesion characterization
NBI can help distinguish adenomatous from hyperplastic patterns and delineate lesion borders before resection. Formal optical-diagnosis strategies require validated classifications, training and quality assurance; a casual visual impression is not equivalent to a validated optical diagnosis.
NBI does not solve every miss
Flat lesions can still be missed if withdrawal is rushed or the colon is poorly prepared. NBI is an enhancement tool, not a substitute for inspection technique or endoscopist quality.
A practical clinical approach
- Begin with clean mucosa and high-definition white-light inspection.
- Activate NBI when a subtle mucosal or vascular abnormality needs characterization or when surveillance protocols support virtual chromoendoscopy.
- Use validated pattern classifications when making optical predictions.
- Biopsy or resect suspicious lesions according to clinical context.
- Document the imaging mode and lesion features when they affect management.
Common errors to avoid
- Using NBI on dirty mucosa and expecting technology to compensate.
- Treating NBI as a histologic diagnosis.
- Skipping systematic Barrett's biopsies solely because no NBI abnormality is visible.
- Failing to train in validated pattern recognition.
What should trainees remember?
NBI helps the trained eye see contrast. It is most useful when it is layered onto excellent inspection and linked to a specific diagnostic decision.
